    • Check vital signs and symptoms.

    • Offer advice on health conditions, treatments.

    • Discuss medicines —both prescription and over-the-counter.

    • To assess the member’s physical, emotional, social, and financial needs.
    •  
    • To assist the member in navigating the healthcare system and help move him/her towards better health and wellness.
    •  
    • Educating the member/family/caregiver about disease processes, self-management, and maintaining wellbeing.

    • Coordinating services and care.

  • Review medical records and reconciling medications.
  •  
  • Communicate with health care providers.
  •  
  • Conduct Home Safety evaluation.
  •  
  • Assess functioning activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IDL).
  •  
  • Assess ability to understand and self-administer medications.
  •  
  • Perform clinical review of history and systems.
  •  
  • Assess health literacy.
